It could be in my pile of mail, unopened. Email me your address and I'll go ahead and send the PCB. Specify thru-hole or SMT.

          Hi Carl,

I started to build HH but in power circuit I have experienced some problems. I am using AC/DC adapter to get 11V for power supply. I am getting voltage readings on IC2 pin1, IC3 pin3, IC4 pin3 the same voltage like power supply 11V or a little less, not +/- 5V like I should.

          What could be the problem?

Put your multimeter ground lead on the circuit ground, i.e., IC1-pin8. Measure -VB (IC1-pin3)... you should have -11V to -12V. If you measure a positive voltage you have the adapter polarity backward.

Now measure IC2-pin3, the input to the 7805. You should read +10V to +11V. If not, IC1 might be bad. Also watch the polarities of IC2, IC3, and IC4. Also cap polarities.
